Carolina Oaks, a division of Tri-County Community Health Council, serving the populations of Johnston, Harnett, and Sampson County. The clinic strives to provide care to our community in a culturally competent, affordable and dignified manner.

Patients are charged on the federal sliding fee scale which means that cost is determined by your salary and your family size. Payment is expected upon completion of your visit and in some cases before services can be offered. Bring in proof of income on first visit to be eligible for the sliding fee scale.
Proofs can include: check stubs from the previous month, W-2 form or a written statement from your employer.
Patients are seen on an appointment or walk-in basis. Walk-ins are seen based on severity of situation.
