Four County Health Center is a division of Tri-County Community Health Council, Inc is a private not-for-profit health center serving the populations of Sampson, Pender, and Duplin County. The clinic strives to provide care to our community in a culturally competent, affordable and dignified manner. The Center is committed to its goal of improved patient health, staff development and community involvement. Our doctors are board-certified, family practice physicians. We are pleased to be able to provide many services onsite and look forward to serving you . Patients are charged on a sliding fee scale which means that cost is determined by your salary and your family size. Payment is expected upon completion of your visit and in some cases (co-pays) before services can be offered. Bring in proof of income on first visit to be eligible for the sliding fee scale. Proof can include: check stubs from the previous month, W-2 form or a written statement from your employer.
Patients are seen on an appointment or walk-in basis. Walk-ins are seen based on severity of situation.
