What are the costs?

At TCCHC, patients are charged based on a sliding scale, which means that your cost is determined by your salaries and your family size. If you would like to seek eligibility for the sliding scale discount, please bring in proof of family income. The center accepts North Carolina Medicaid, Medicare, Health Choice and most private insurances. Please bring your insurance card with you for each visit. It is the patient's responsibility to pay the deductible, co-pay or any balance not covered by the insurance. You will be refferred to a financial counselor, feel free to ask any questions that you may have about your insurance coverage and fees .

What are Financial Counselors?

Financial Counselors are members of the Patient Registration Team who are dedicated to helping patients and physicians determine sources of reimbursement for clinical services.

As a private, non-profit institution, Tri-County Community Health Council’s clinics relies on patient care revenues, whether from commercial insurance, government programs, or direct patient payments.

Financial Counselors identify and assist patients who are unable to pay their estimated liability prior to treatments or who have large existing balances.

This assistance includes:

  • Assessing a patient's liability
  • Social Service Referrals

Patients requesting payment plans or financial hardship consideration are identified as early as possible in the registration/admission process so the necessary documentation, research, and approval can be completed before services are provided.

As a general policy, patients seeking elective/non-urgent services must have an identified financial source for covering the cost of care. This policy does not apply to persons who seek services in the Emergency Department until after the patient has been medically screened for an emergency medical condition and has either

  • (a) been determined not to have an emergency medical condition
  • (b) been provided with stabilizing treatment.

In compliance with COBRA regulations, Tri-County Community Health Council’s Clinics will provide a medical screen examination and necessary stabilizing treatment for emergency medical conditions and labor regardless of ability to pay.

Medical Records

Medical Records – Release of Information

  • A patient, or his/her legal representative, may inspect and/or obtain a copy, or have copies of medical records sent to another facility.
  • Tri-County Community Health Council’s   Clinics requires a completed and signed written request and authorization for release of health information form before releasing any documents to anyone, including the patient.
  • In certain cases, a patient’s physician, psychologist or social worker may also be required to approve a request.

To Request a Copy of Your Medical Records

  • The form must be completed, dated and signed
  • We ask that you specify what components of your medical records you wish to obtain.  Often, the discharge summary, operative report and history and physical contain relevant information to suit your needs.
  • Requests must be specifically signed if requesting/authorizing the following information:
    o Psychiatric Care
    o AIDS/HIV
    o Alcohol/Drug abuse
    o Fertility Treatment

Release of Information Charges

  • $0.25 per page for copying plus postage and handling
  • If your charges exceed $50, we require payment before the records are released.
  • There will be no charge if request is going to another health care facility to maintain continuity of care.

Additional Information

  • If an individual other than the patient is picking up the records, then that individual must have an original signed authorization letter from the patient and along with a photo ID.
  • Please allow up to 15 working days for your request to be processed.  If you indicated the option to pick-up, you will be contacted by the Release of Information Office when your records are ready.  A photo ID is required .
